=== Lactic acidosis === A randomized controlled trial in children with congenital lactic acidosis found that while DCA was well tolerated, it was ineffective in improving clinical outcomes. A separate trial of DCA in children with MELAS (a syndrome of inadequate mitochondrial function, leading to lactic acidosis) was halted early, as all 15 of the children receiving DCA experienced significant nerve toxicity without any evidence of benefit from the medication. A randomized controlled trial of DCA in adults with lactic acidosis found that while DCA lowered blood lactate levels, it had no clinical benefit and did not improve hemodynamics or survival. Thus, while early case reports and pre-clinical data suggested that DCA might be effective for lactic acidosis, subsequent controlled trials have found no clinical benefit of DCA in this setting. === Aortic regurgitation === Chest X-ray is not as sensitive as other tests, but it may show aortic root dilation (especially in causes involving the aortic root) and apex displacement. An ECG may show left ventricular hypertrophy and signs of left heart strain. Left axis deviation can be a sign of advanced disease. An echocardiogram can be helpful in determining the root cause of the disease, as it will clearly show aortic root dilation or dissection if it exists. Typically the pump function of the heart during systole is normal, but an echocardiogram will show flow reversal during diastole. This disease is classified using regurgitant fraction (RF), or the amount of volume that flows back through the valve divided by the total forward flow through the valve during systole. Severe disease has an RF of >50%, while progressive aortic regurgitation has an RF of 30–49%.

== Life == Theodore Ryder was born in Keyport, New Jersey, in 1916. At the age of four, he developed symptoms of diabetes mellitus, including greatly increased urine output, constant, intense thirst and significant weight loss. At that time, no therapy was available to treat diabetes, so the disease would have led to death within a short period of time. The only treatment option was a strict low-carbohydrate low-calorie diet of as little as 500 calories per day, which extended the lives of affected patients by about one to two years. In the spring of 1922, the physicians Frederick Banting and Charles Best at the University of Toronto succeeded in producing insulin purely from extracts of pancreatic tissue. After this became known to Theodore Ryder's family, an uncle who worked as a doctor in New York City attempted to have his nephew included in experiments to test insulin through a personal conversation with Banting. After initial hesitation, Banting gave in and began treating the boy on July 10, 1922, who at that time weighed only 12.5 kilograms at the age of five. Within a few months, Theodore Ryder made a full recovery and became a symbol for the staff treating him of the dramatic successes brought about by insulin. In October of the same year he was able to return home with his mother. In a letter of thanks to Banting he wrote:Dear Dr. Banting, I wish you could come to see me. Palominos have a chestnut base coat color that is genetically modified to a golden shade by a single copy of the incomplete dominant cream gene.
